RN Care Coordinator:
Under supervision of the Nursing Services Manager, the Senior RN Care Coordinator is responsible for, but not limited to, day to day site and service supervision, coordination of case management, supervision of nursing and services staff, and provision of care management and skilled nursing services.
The Senior RN Care Coordinator will comply with all agency policy and procedures, as well as recommend employee development.
